This session, led by Dr. Brad Bahler, will set the stage by describing the current Alberta primary care landscape and the design for the future. He will invite participants to take on the mission of ‘change agent’- an essential role in supporting primary care transformation.
The change agent will guide the primary care team in Patient’s Medical Home (PMH) practice transformation. Your intel will include key messages to share with TARGETS and the skills you need to accomplish the mission. Tuxedo not included!
The Desire is Not Enough - Chris Wise, Ph.D., M.H.A
Chris Wise, PhD.,M.H.A., has worked with many organizations who are successfully implementing the PMH. He will share the lessons learned from those who are ahead of us in the journey, including the importance of building the adaptive reserve that is necessary to support teams’ ongoing transformation in the primary care setting.
Multiple tools and strategies will be added to your to your ‘attaché case’, such as value stream mapping.
