Our Role in PCN Evolution
TOP is one of many supports available to PCNs to engage their member practice teams in the work of the Patient's Medical Home - i.e. PCN Evolution. Our focus is to help PCNs and practice teams establish a strong foundation upon which teams can continue to build the best medical homes for their patients and themselves. Foundational elements of focus include:

Alberta Peer to Peer EMR Network Program
The program specifically focuses on sharing EMR expertise related to planned and proactive panel-based care in a Patient’s Medical Home. TOP engages physicians and team members as peer leaders to share their EMR processes for panel identification, maintenance and management. Building Engaged Physician Leadership
Transformation to the Patient’s Medical Home will entail major changes to a practice’s culture, relationships and routines. Engaged leadership will be a key enabler to facilitate the work of change at the practice and system level. Building Capacity for Improvement
The Patient's Medical Home is not a one-size-fits-all implementation approach - i.e. each practice team’s transformational journey will be different. Panel Knowledge & Skill Development
At the heart of the Patient's Medical Home is the relationship between the patient, their physician and practice team who provide and coordinate their care. The term panel refers to a list documenting this relationship, whether formal or informal.
